Section Report Form

All leadership teams should submit a report summarizing activities since the last reporting period. These reports are due twice a year, in December and May.

Your report will be posted to this site publicly, so please do not include any confidential or personal information in your answers.

Once submitted, your report will be sent to the Core Presidents, Executive Director, your staff liaison, your Board liaison, the chair of the Diversity and Inclusion Committee, and you.
